What is HHSC Form 4122?
Form 4122 is the HHSC service delivery log for HCBS Host Home / Companion Care. The companion provider records the services delivered to the individual living in their home on a weekly log, and the agency keeps it as the documentation evidence for that service. In Sereniq the host-home provider completes it from the app and it prints to the HHSC layout by week or by calendar month.
