Lina clocks in at Maple Court

Lina, a direct support professional (DSP), opens Sereniq on her phone and taps Clock in. Sereniq checks that she is at the home, and the time comes from Sereniq, not from the phone’s clock.

Once she is on shift, her phone shows the individuals who live at Maple Court and no one else.

The morning med pass, charted as it happens

Lina charts each of B. Thomas’s 8:00 AM doses on the MAR as she gives them. He refuses his Metformin, so she marks it refused and writes what he said.

Every tap fills in the monthly Form 3092 grid, so nobody copies the MAR onto paper at the end of the month.

The refusal reaches the supervisor and nurse right away

The moment Lina saves the refusal, Sereniq sends a critical alert to the agency’s supervisors and nurses, linked to B. Thomas’s MAR.

Nobody has to remember to call. The supervisor and nurse open the MAR, read Lina’s note, and decide whether the follow-up needs an incident report.

She types what happened in her own words, and AI polish tidies the wording without adding facts. Lina reads the result and fixes anything before she submits, because a person reviews AI output before it lands in the record.

Maple Court is an HCBS Supervised Living home, so the note links to one of J. Alvarez’s active IRP goals before it can be submitted.

The supervisor sees what needs attention first

The house supervisor opens Sereniq on the web. The dashboard puts the urgent items first: the morning’s refusal, notes waiting for review, recent incidents, and documents about to expire.

She approves Lina’s note or sends it back for edits. Directors get the same view across every home they run.

The director asks the whole record a question

The director presses Cmd+J and asks, in plain English, what still needs attention at Maple Court. Ask Sereniq answers from the agency’s own records, and only for homes she can already see.

Ask Sereniq can also start work, like a survey-prep bundle, but nothing runs until a person confirms, and a person reviews AI output before it lands in the record.

Form 4119 in 20 seconds, not 30 minutes

When the week closes, the supervisor prints Form 4119. Sereniq builds the service delivery log from the week’s approved notes, with each staff member’s initials under the right day.

HCS homes get the daily per-shift log and HCBS Supervised Living homes get the weekly one. Host Home providers print Form 4122 instead.

Does the day look the same for HCS, HCBS, and Host Home?

Mostly, yes. The phone app, the MAR, the refusal alert, the review queue, and Ask Sereniq work the same in every program. What changes is the service delivery log, the care plan rule, and whether there is a shift to clock into, and Sereniq sets those from each individual’s program.
