Form 8578 is four pages of coded boxes: purpose of assessment, legal status, previous residence, level of care, level of need, adaptive behavior level, instruments used, and the nursing, behavior, day, and employment items that decide the LON. Filled by hand, it is easy to enter a code that does not exist, miss the renewal date, or lose track of which version the state actually approved.

Level of care is the eligibility finding, an intellectual disability or a related condition. When does Sereniq remind us that an ID/RC is expiring?

60 days, 30 days, and 7 days before the state expiration date on the form, or before the requested end date if the state result is not recorded yet. Reminders go to administrators and HR in the app, and they clear when the renewal is finalized or the state result is recorded.

Does the fillable ID/RC replace the signed copy we already file?

No. The signed copy stays as an upload, and the binder shows the fillable form beside it. Use the record to fill and print the form, collect signatures, then upload the scan.
