Individualized Skills and Socialization replaced day habilitation in Texas in 2023, and it came with its own license, its own staffing ratios for on-site and off-site time, and its own weekly service delivery log on HHSC Form 8615. Most ISS programs still run the day on a paper sign-in sheet, rebuild the 8615 by hand on Friday, and total the month for each placing agency in a spreadsheet. A day with no activity initialed is a survey finding, and a month with one missing checkout bills nothing for that person.

Time records on the weekly service log come from the board and are never typed twice. Staff initial the activities they helped with, the log shows its 14-day deadline, and it prints as a one-to-one Form 8615 facsimile with the on-site and off-site sections, ratios, and provider names filled in. Drills print on Form 4719, and incident reports go to a review queue with a nurse alert when emergency services were involved.
Billing is a checklist, not a spreadsheet. Sereniq builds the month from the attendance board, trims days to the daily cap, lists what to fix before the month can close (someone never checked out, an agency with no rates set), and produces one statement per placing agency with Ready, Sent, and Paid status. Closing the month locks it. If your agency bills the state for its own individuals, the hours report gives you the totals. Sereniq does not submit Medicaid claims.
