Time in and time out come from the attendance board and are never typed twice. Staff tick the activities the group did, write the day once, and each person’s log fills in. Every log shows its 14-day deadline, and the print is a one-to-one facsimile of HHSC Form 8615.
Day done is where the log gets written. Staff tick what the group did once, from the activity grid on the state form (socialization, self-help, adaptive, and implementation plan skills), and write the group note once with the word Name where each person’s name goes. Each person then gets their own summary to adjust. Initials are bound to the staff member who made them, so nobody can initial a cell for someone else.
The week grid shows what is owed, not just that something is. One row per person with five day marks: came and initialed, came and still needs initials, expected and did not come, not their day. The deadline is 14 calendar days after the first attended day of the week, and every log reads Due in N days, Due today, Overdue by N, or Complete, so the Friday packet is never a surprise. Managers and the staff who worked that week are reminded three days before a log is due, and again when it is overdue.
The print is the June 2026 Form 8615, page for page: the HHS lockup, both the on-site and off-site sections, two time in and time out blocks per day, and on off-site rows the ratio, the service provider’s name, and the number of people. Community locations and special occurrences are on the form. The daily summary prints behind the facsimile on an agency-branded page, never on it.
