Drills and incidents

Drills are timed alarm to rally point. A fire drill over the program’s limit is refused before it is saved, so staff run it again, and each one prints on HHSC Form 4719 with day program locations (dining, laundry, office) and counts against the monthly target the program sets. Hurricane and tornado drills are recorded the same way. Incidents use the standard report with one extra tick, emergency services were involved, which marks the alert critical. The program nurse is told of every incident filed, open and late reviews sit in the Needs you list until a manager signs off, and a filed incident can never be deleted.

The behavior record

The Maladaptive Behavior Record is laid out like the paper form, with a manager sign-off and a one-page print. Ticking a restraint item tells staff that an incident report and a restraint record are also needed and rings the managers and the program nurse. It never blocks the save and is never a substitute for a reportable incident.

Individuals and the agencies that place them

Every person has a face sheet (identity, demographics, medical profile, allergies, diet, mobility, communication, emergency contacts), a placing agency, a level of need with the staff ratio that follows it, expected days, and a document binder with the kinds the program expects: consent packet, PDP, face sheet, medication list, in-service note, behavior support plan. Social Security, Medicaid, and Medicare numbers are encrypted and masked, revealed only by admins and supervisors, with every reveal audited. Each partner agency has its own record: address, license number, case manager, nurse, enrollment date, service agreement, and the people it places. A residential HCS provider enrolls its own individuals from its homes in one step.
