Upload the IRP or PDP and confirm the goals once. The same goals appear on the DSP’s mobile note the moment they start writing, and the DSP picks which ones the shift touched. No re-typing on every shift, and that is the linkage HHSC asks for on every survey.
For HCS, the four planning documents sit on one Plan of care tab: the ID/RC, the PDP, the IPC, and the Implementation Plans. A four-step walk builds next year’s plan, a first plan, or a change: renew the ID/RC, read the new PDP once (a program manager accepts or edits each row), build the Form 3608 IPC and a Form 2125 per service, then check and sign. Units never come from the PDP, and a packet check names every mismatch as advice, never a block.
The IPC, the Implementation Plans, and ID/RC item 56 are signed in Sereniq. Staff sign with a saved signature, the individual and family members sign in person on a device, and the service coordinator or LAR can sign from a 72-hour emailed link sent to the address on the record. Prints carry each signature plus a Signature record page, and a signed paper copy can still be attached.
Same as last year renews a plan with new dates and sends it straight to signatures. The expiry tracker flags IRPs and PDPs at 30 days, 7 days, and on expiry, and the ID/RC at 60, 30, and 7 days, with a notification fan-out to the right roles. The service coordinator’s contact is on the individual record, so when a plan needs renewing you know who to call.
