Nurses open the phone app to My individuals: everyone in their homes, host homes included, grouped by home and tagged Draft to sign, No visit this month, or Last visit. Tap Start when the visit begins and Stop when it ends, and the times fill in on their own. With no signal, the log saves on the phone and sends itself when the phone is back online, with no double entries. The phone app speaks English and Spanish.
The activity list has 22 plain-word items, each tied to its Form 4123 row: give a medicine the doctor ordered, a health check, the comprehensive nursing assessment (CNA), teach staff a delegated task, talk with a pharmacist, help write the IPC or the implementation plan, the annual nursing report, and more. RN-only items stay locked for an LVN, and phone and video visits grey out what does not count that way. Units of service are worked out from the state table, with the 8-minute floor and the time split fairly when one nurse serves several people.
Before signing, Sereniq checks the log. A word-for-word copy of the nurse’s last log for that individual is refused. Vague words like “had a good day” or “same as above” get a warning, and so does a type of nursing that is not on the individual’s current IPC. A log written more than 14 days after the visit can still be signed and prints with a late stamp; a visit more than 120 days old cannot be signed. Signed logs are locked. A mistake is fixed with a correction on the web, and both print.
The office sees nurse time per individual for the month on the Nursing page, with every active HCS individual who has had no nurse visit flagged No visit yet. The print is the state form plus an Attachment to Form 4123 page for the medical need, anything unusual, and the late or correction details the state form has no box for.
